PARTICIPANT LIABILITY WAIVER & RELEASE

READ THIS DOCUMENT CAREFULLY AS IT AFFECTS YOUR LEGAL RIGHTS

In consideration of the undersigned Participant’s participation in the parenting class (the “Class”) being provided by
Thresholds (“Company”) on the Class Date at the Class Location, the Participant, on behalf of themselves, their heirs and legal
representatives, hereby expressly agree to the following:

Participant acknowledges that participation in the Class may involve physical activity, including but in no way limited to,
bending, lifting, squatting, sitting, pulling, standing, grasping, stretching, balancing, and bearing the weight of another person.
Participant also acknowledges that performing physical activities such as these may subject the Participant to harm or injury.
Participant further acknowledges that the Class may cover emotionally driven content that may be triggering. Notwithstanding
the inherent risks, Participant has chosen to attend and/or participant in the Class and expressly, knowingly, voluntarily,
unconditionally and willingly assumes any and all risk of personal injury, bodily injury, illness, death, (collectively, “Harm”)
arising out of, resulting from, in connection with and related to Participant’s attendance and/or participation in the Class.

Participant represents to Company that they are physically and mentally sound and suffering from no condition, impairment,
disease, infirmity or other illness that would prevent their participation in the Class. Participant is solely responsible for knowing
their limitations and tailoring their participation in the Class accordingly.

Participant hereby RELEASES, ACQUITS AND FOREVER DISCHARGES, and WAIVES any and all past, present and future
claims, losses, costs (including, but not limited to, costs of defense or settlement, attorneys’ fees and court costs), expenses,
liabilities, demands or causes of action (collectively referred to as the “Claims”), from and against Company and all of Company’s
owners, officers, employees, agents, or representatives (hereinafter, “Representatives”) (collectively with Company, the
“Indemnified Parties”) that arise out of, result from, are in connection with, or that relate to Participant’s attendance or
participation at the Class and Participant agrees not to sue any of the Indemnified Parties for such Claims or any Harm in
connection therewith. The releases, waivers and indemnities contained in this Agreement expressly shall apply regardless of
whether the Claims to be released, waived or indemnified against or Harm in connection therewith arise, or are alleged to arise,
from the NEGLIGENCE (WHETHER SOLE, JOINT OR CONCURRENT), GROSS NEGLIGENCE, NEGLIGENCE PER SE,
and/or STRICT LIABILITY of any of the Indemnified Parties.

PARTICIPANT AGREES TO INDEMNIFY, DEFEND AND
HOLD HARMLESS THE INDEMNIFIED PARTIES FROM AND AGAINST ANY AND ALL CLAIMS ARISING OUT
OF, RESULTING FROM, RELATING TO OR IN CONNECTION WITH PARTICIPANT’S ATTENDANCE OR
PARTICIPATION IN THE CLASS. THE INDEMNIFIED PARTIES SHALL NOT BE LIABLE TO PARTICIPANT,
PARTICIPANT’S HEIRS, REPRESENTATIVES, SUCCESSORS AND ASSIGNS FOR ANY HARM ARISING OUT
OF, IN CONNECTION WITH, RESULTING FROM OR RELATED TO THE CLASS.

In executing this Participant Liability Waiver and Release I am not relying upon any oral or written representation or statements made by the Company or any Company Representatives other than what is set forth herein.

I have read and understand this document and I am aware that by signing below I am waiving certain legal rights, including the right to sue, which I, or my heirs, next of kin, executors, administrators and assignees, may have against the Company or any of Company’s Representatives.

VIDEO RELEASE FORM

I hereby grant permission to Thresholds, the rights of my image, in video or still, and of the likeness and sound of my voice as 

recorded on audio or video tape, without payment or any other consideration. I understand that my image may be edited, copied, 

exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. 

Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also 

understand that this material may be used in diverse educational settings within an unrestricted geographic area.

Photographic, audio or video recordings may be used for ANY USE which may include but is not limited to:
Presentations;
Courses;
Online/Internet Videos;
Media;
News (Press);

By signing this release, I understand this permission signifies that photographic or video recordings of me may be electronically 

displayed via the Internet or in the public educational setting.

I will be consulted about the use of the photographs or video recording for any purpose other than those listed above.

There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

This release applies to photographic, audio or video recordings collected as part of the sessions listed on this document only.

By signing this release, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational purposes.
